pet ID kit folds into four sections, size: 4.25"
x 5.5".

(1) vital physical, (2) medical, (3) identifying information,
(4) photograph, (5) shots and (6) vet information, (7) emergency
response measures if pet is missing and (8) a " missing pet
" flyer.
- Each
pet identification kit is inserted into a plastic sleeve.
- Professionally
